Medical Photography

By signing this form, you authorise Kinvara Private Hospital and its clinical staff, including your Consultant and nursing team, to take and store photos, video, and/or audio recordings of your body.

I understand that:

• Photographs, video, and/or audio recordings may be taken of me or parts of my body before, during, and after surgery by my Consultant, nursing staff, or other authorised clinical personnel.
• My identity will not be revealed unless I give explicit consent.
• My photographs, video, and/or audio recordings may be shared with the public, doctors, or health professionals through the following media: medical publications, medical journals, medical textbooks, Kinvara Private Hospital literature, the Kinvara Private Hospital website, social media platforms (including Instagram, Facebook, and Twitter), and commercial media such as newspapers and television, for marketing and/or educational purposes.
• Refusal to consent to photographs, video, and/or audio recordings will not affect my medical care.
• Although photographs, video, and/or audio recordings may be used without identifying information, it is possible that I may still be recognised.
• I may request a copy of, or access to, these photographs, video, and/or audio recordings. Kinvara Private Hospital reserves the right to edit such materials to preserve the privacy of staff and other patients, or to protect commercially sensitive information. I will only have access to such edited versions.
• All photographs, video, and/or audio recordings remain the property of Kinvara Private Hospital and are held in accordance with the Data Protection Act 2018.
• My participation is voluntary, and I agree that the use of any photographs, video, and/or audio recordings confers no rights of ownership or royalties whatsoever.
• I may withdraw my consent at any time by writing to Kinvara Private Hospital.
• Withdrawal of consent will take effect from the date it is received by Kinvara Private Hospital. Any photographs, video, and/or audio recordings already in the public domain at that point cannot be withdrawn.
• Once photographs, video, and/or audio recordings have been shared through publications, websites, social media, and/or commercial media, Kinvara Private Hospital cannot control their further dissemination.
• I hereby release Kinvara Private Hospital, its employees, and any third parties involved in the creation or publication of educational or marketing materials from liability for any claims by me or any third party in connection with my participation.
• I have read this consent form in its entirety, or it has been read to me. I have had the opportunity to ask questions and I understand its contents.
