Patient Safety Incident Response Framework
Our commitment to patient safety and continuous improvement.
Last updated January 2026
Introduction
The National Health Service (NHS) has introduced a new approach to handling Patient Safety Incidents, known as the Patient Safety Incident Response Framework (PSIRF). This framework replaces the Serious Incident Framework (2015), and marks a significant change in how patient safety incidents are addressed.
At Kinvara Private Hospital, we have fully adopted this new framework and now manage incidents in accordance with its guidelines.
Key Objectives of the PSIRF
The Patient Safety Incident Response Framework aims to:
- To foster more compassionate engagement with those affected by patient safety incidents.
- To provide staff with opportunities for reflection.
- To gain deeper insights into the root causes of incidents.
By focusing on understanding how incidents occur, PSIRF enables us to extract valuable lessons and implement improvements. This approach contributes to the development of a safer healthcare environment for our patients.
