You can submit completed forms electronically through the links provided, or bring printed copies with you to your appointment. If we have your email address, we can also send forms to you through DocuSign for electronic signature.
We recommend completing all relevant forms at least 48 hours before your appointment. This allows our team to review your information in advance and makes your visit more efficient.
The forms you need depend on the type of treatment or surgery you are having. All new patients should complete the New Patient Registration & Agreement. Your consultant's office will inform you of any additional forms specific to your procedure.
