Trigger thumb (stenosing tenosynovitis of the flexor pollicis longus) is a common hand condition in which the flexor tendon catches as it tries to glide through a tight A1 pulley at the base of the thumb. Patients notice a painful click or snap, the thumb may lock in a bent position, and a small tender nodule can often be felt in the palm at the base of the thumb. It is more common in women, in people aged 40 to 60, and in patients with diabetes.

What is Trigger Thumb?

Your thumb is controlled by the Flexor Pollicis Longus (FPL) tendon, which runs through a series of tunnels called pulleys. The first of these, the A1 pulley, sits at the base of the thumb. In trigger thumb, inflammation causes the tendon to swell or the pulley to thicken.

As the swollen tendon tries to pass through the narrowed pulley, it catches. This mechanical 'mismatch' causes the characteristic snapping sensation. Unlike the other four fingers, the thumb only has two joints, making a trigger thumb particularly disabling for tasks requiring a 'pincer' grip or fine motor skills. While steroid injections can provide temporary relief, surgical release is often required to permanently widen the tunnel and allow for frictionless movement.
